Founded in 1992, Community Health Plan of Washington (CHPW) is a not-for-profit health plan offering Medicaid, Medicare Advantage, and Individual & Family health plans in Washington state. A cornerstone of the state’s healthcare safety net, CHPW is committed to providing whole-person, integrated care: addressing physical and behavioral health, as well as social drivers of health outcomes such as housing, food availability, and access to transportation. The CDFI Fund Certified CDFI point dataset has been updated for their latest January release.
